How you found us: the link or QR code that brought you here (for example campaign tags in the web address), the page you landed on, and the website you came from.
Basic technical data: your IP address and browser type, which are received when you submit the form and used for security and to prevent spam.

What we do not ask for

We do not ask for health information on this website, and the form has no health questions. Please do not include symptoms, conditions, medications or other health details in the form. A conversation about your health is for you and a licensed physician.

To contact you about your inquiry by phone, text and email.
To tell you whether services are available in your state, and to keep a waitlist for other states.
To understand which links and events bring people to the site, so we can improve it.
To protect the site from spam and misuse, and to meet legal obligations.

Who we share information with

Service providers that help us run the program, such as our customer-relationship and messaging platform (GoHighLevel) and our website host (Cloudflare). They may use your information only to provide services to us.
Our independent physician partner, only if you ask to be connected, and only the contact details you gave us.
Authorities or advisers when the law requires it or to protect rights and safety.

We do not sell your information.

Website tracking

We keep tracking minimal. To remember how you found us while you browse, this site stores the campaign details from your link in your browser's temporary session storage, which is cleared when you close the tab. We do not use that information to learn anything about your health.

Keeping your information

We keep your information for as long as we need it for the purposes above, then delete or anonymize it.

Security

We use reasonable measures to protect your information. No website or service is perfectly secure, so we cannot guarantee absolute security.
